By accessing or using the website, health services, mobile applications, or patient portals of Precision Neuro Recovery (collectively, the “Services”), you agree to be bound by these Terms of Service (“Terms”). Acceptance is indicated by actions such as creating an account, scheduling an appointment, or using the Services. If you do not agree to these Terms, you may not use the Services.

Precision Neuro Recovery (“PNR”) provides specialized neurological care, including but not limited to clinical consultations, diagnostic evaluation, treatment planning, and medication management, through appropriately licensed healthcare providers during authorized health sessions. Care is delivered in accordance with applicable professional standards and within the scope of each provider’s licensure. The Services are not intended for medical emergenciesor situations requiring immediate, in-person intervention. If you experience a medical emergency, sudden neurological deterioration, or any condition requiring urgent care, you should immediately contact 911 or proceed to the nearest emergency department.
You agree to use the Services only while physically located in a state where PNR providers are licensed to practice at the time care is rendered. PNR currently provides services in jurisdictions where its providers are duly licensed and authorized to practice, and may expand to additional states in the future as permitted by law. The list of licensed jurisdictions is maintained and updated at https://precision-neurorecovery.com/. Unauthorized use of the Services—including accessing care from an unlicensed jurisdiction, providing inaccurate or incomplete health information, or interfering with communications or data systems—may result in suspension or termination of access.
By using the Services, you consent to receiving neurological care and acknowledge the potential benefits and risks associated with such care. Certain Services may be provided through telehealth or other technology-enabled modalities, which may involve limitations when compared to in-person encounters. When receiving Services via telehealth, you are responsible for maintaining adequate internet access, compatible devices, and functionality necessary to participate through PNR’s communication platforms.
PNR is not responsible for outcomes resulting from patient non-adherence to treatment recommendations, inaccurate information provided by the patient, or circumstances beyond PNR’s reasonable control, including internet disruptions or service interruptions involving HIPAA-compliant third-party platforms (such as healthcare software or SMS delivery systems). Services provided pursuant to a medical lien agreement may involve delayed payment and coordination with legal counsel, as further described in Section 18.
PNR’s care team includes licensed neurologists and other qualified clinical providers, who deliver services within their respective scopes of practice and licensed jurisdictions. By using the Services, you acknowledge that PNR providers are appropriately qualified to deliver the neurological care offered.

Precision Neuro Recovery is required by law to maintain the privacy and security of your health information through strict administrative, technical, and physical safeguards. However, clinical services are provided by licensed professionals in accordance with applicable standards of care.
13. Limitation of Liability
Precision Neuro Recovery, its affiliates, and their officers, directors, employees, or agents shall not be liable for any direct, indirect, incidental, special, consequential, or punitive damages arising from:
(a) Use or inability to use the Services, including technical failures;
(b) Website or healthcare platform content; or
(c) User conduct.
14. Indemnification
You agree to indemnify and hold harmless Precision Neuro Recovery, its affiliates, and their officers, directors, employees, and agents from claims, damages, or costs arising from your intentional misuse of the Services or violation of these Terms, except where such claims result from PNR’s negligence or misconduct.
15. Governing Law
These Terms shall be governed by and construed in accordance with the laws of the State of Arizona, without regard to its conflict-of-laws principles. PNR currently provides healthcare services through licensed providers in Arizona, Georgia, and Tennessee. To the extent that healthcare services are provided to a Patient while the Patient is physically located in one of these or any other jurisdictions where PNR providers are duly licensed, the applicable healthcare and professional-practice laws of the state in which the Patient is located at the time services are rendered shall apply, as required by law.
PNR may expand its services to additional states in the future, and these Terms shall be interpreted and applied in a manner consistent with all applicable state and federal healthcare laws and regulations.
16. Patient Location Acknowledgment:
By using PNR’s healthcare services, you acknowledge and agree that you must be physically located in a state where PNR providers are duly licensed at the time services are rendered. PNR currently provides services in Arizona, Georgia, and Tennessee, and may expand to additional states as permitted by law.
You are responsible for promptly notifying PNR of any change in your physical location before or during a scheduled appointment, as your location determines whether services may legally be provided. If you are located in a jurisdiction where PNR providers are not licensed at the time of service, PNR may be required to delay, modify, or discontinue services in order to comply with applicable state licensing and professional-practice laws.
17. Medical Liens
If you receive healthcare services under a medical lien agreement, you authorize PNR to share health and billing information with your attorney or other parties involved in your legal action, as permitted by HIPAA and outlined in the lien agreement. Such sharing supports payment for services via settlement, judgment, or verdict proceeds.
The Services may involve electronic agreements, including these Terms or medical lien agreements. Electronic signatures shall have the same legal effect as original signatures, in accordance with the Electronic Signatures in Global and National Commerce Act (E-SIGN) and applicable states laws.
18. Changes to These Terms
PNR reserves the right to modify, update, or revise these Terms at any time, as permitted by applicable law. Any changes will become effective upon posting the updated Terms on https://precision-neurorecovery.com/or through applicable patient portals, unless a later effective date is expressly stated.
Your continued use of the Services after such changes constitutes acceptance of the revised Terms. If you do not agree to the updated Terms, you should discontinue use of the Services and contact PNR regarding your care options.

By opting into SMS communications, including but not limited to those through our website, patient portal, or health services, you agree to receive SMS messages from Precision Neuro Recovery. These messages may include appointment scheduling, appointment reminders, post-visit instructions, lab notifications, and billing notifications. Message frequency varies based on your treatment needs and interactions with PNR. Message and data rates may apply, as determined by your mobile carrier.
